Sample report on a fictional product

How this sample was made

This page shows the format of a report. No product was tested and no person tested anything for it. ExampleRec Registration does not exist, and every level, remark, date and measurement below is invented.

The page was drafted by an AI assistant (Claude) for Novruz Veliiev. A script compared its 50 criteria with W3C's WCAG 2.1 Recommendation, version of 6 May 2025, on 5 November 2026. The template wording follows a vendor's published report based on VPAT 2.5Rev, not ITI's own file.

A paid report is different. A person tests the real product by hand with the methods listed under Evaluation Methods Used, and each remark describes what happened.

Terms

The terms used in the Conformance Level information are defined as follows:

WCAG 2.x Report

Note: When reporting on conformance with the WCAG 2.x Success Criteria, they are scoped for full pages, complete processes, and accessibility-supported ways of using technology as documented in the WCAG 2.0 Conformance Requirements.

Rows in this sample by level: Partially Supports: 20, Not Applicable: 5, Supports: 22, Does Not Support: 3. WCAG 2.2 rows are left out because the report does not cover WCAG 2.2.

Table 1: Success Criteria, Level A

Fictional product. The rows illustrate format only.

Notes: rows cover the web screens S1 to S14 and the iPhone screen S15.

Table 1: Success Criteria, Level A. Fictional product; the rows illustrate format only.
CriteriaConformance LevelRemarks and Explanations
1.1.1 Non-text Content (Level A)Conformance Level: Partially SupportsRemarks and Explanations: S9 site map: the map image has the alt text "map" and the pins have no names. S13 calendar: the open and booked icons have no text alternative. Other images in the sample have suitable alt text.
1.2.1 Audio-only and Video-only (Prerecorded) (Level A)Conformance Level: Not ApplicableRemarks and Explanations: The sampled screens have no prerecorded audio-only or video-only content.
1.2.2 Captions (Prerecorded) (Level A)Conformance Level: SupportsRemarks and Explanations: S1 welcome video, 1 min 40 s: captions are present and match the speech for the full length.
1.2.3 Audio Description or Media Alternative (Prerecorded) (Level A)Conformance Level: SupportsRemarks and Explanations: S1 welcome video: a transcript under the player includes the visual steps shown on screen.
1.3.1 Info and Relationships (Level A)Conformance Level: Partially SupportsRemarks and Explanations: S14 staff roster: the table is built from div elements, so NVDA reads no row or column headers. S5: the required marker comes from CSS content and is not tied to the field. Headings and lists on other screens are correct.
1.3.2 Meaningful Sequence (Level A)Conformance Level: SupportsRemarks and Explanations: Reading with NVDA in browse mode follows the visual order on all web screens, including the two-column layout on S3.
1.3.3 Sensory Characteristics (Level A)Conformance Level: Partially SupportsRemarks and Explanations: S8 waiver: the only instruction is "use the green button on the right". Other screens name the control in the instruction.
1.4.1 Use of Color (Level A)Conformance Level: Partially SupportsRemarks and Explanations: S13 calendar: booked days are red and open days are green, with no other cue. Links in body text elsewhere are underlined.
1.4.2 Audio Control (Level A)Conformance Level: Not ApplicableRemarks and Explanations: No audio plays by itself on any sampled screen. The S1 video starts only when the user presses play.
2.1.1 Keyboard (Level A)Conformance Level: Partially SupportsRemarks and Explanations: S9 registration step 3: the map pin cannot be placed without a mouse, and the address field offers no equivalent. S13: calendar cells respond to a click only. All other sampled controls work from the keyboard.
2.1.2 No Keyboard Trap (Level A)Conformance Level: SupportsRemarks and Explanations: Focus can be moved away from every sampled control with Tab, Shift+Tab or Escape, including the S9 map and the S13 date picker.
2.1.4 Character Key Shortcuts (Level A 2.1 and 2.2)Conformance Level: Not ApplicableRemarks and Explanations: The product has no single-character keyboard shortcuts. Letters, numbers and punctuation pressed on S2 and S12 with focus on the page did nothing.
2.2.1 Timing Adjustable (Level A)Conformance Level: Partially SupportsRemarks and Explanations: S10 payment: the session ends after 10 minutes with no warning and no way to extend it. The signed-in session on S12 warns at 2 minutes and can be extended.
2.2.2 Pause, Stop, Hide (Level A)Conformance Level: SupportsRemarks and Explanations: S1: the rotating banner has a visible pause button that works from the keyboard. No other content moves by itself.
2.3.1 Three Flashes or Below Threshold (Level A)Conformance Level: SupportsRemarks and Explanations: Nothing flashes on the sampled screens. The S1 video has no flashing on a full viewing.
2.4.1 Bypass Blocks (Level A)Conformance Level: SupportsRemarks and Explanations: A skip link is the first focus stop on every web screen and moves focus to the main content. Landmarks are present.
2.4.2 Page Titled (Level A)Conformance Level: Partially SupportsRemarks and Explanations: S7, S8 and S9 all have the title "Register", so the step cannot be told from the title. Other screens have unique titles.
2.4.3 Focus Order (Level A)Conformance Level: Partially SupportsRemarks and Explanations: S6: when the Add participant dialog closes, focus goes to the top of the page and not back to the button. Focus order is logical on other screens.
2.4.4 Link Purpose (In Context) (Level A)Conformance Level: Partially SupportsRemarks and Explanations: S2: every card ends with a link named "Details", which is not in the same list item as the program name. Other links are clear from their text.
2.5.1 Pointer Gestures (Level A 2.1 and 2.2)Conformance Level: SupportsRemarks and Explanations: S9 map: pinch to zoom also has plus and minus buttons that work with one finger. No other path or multipoint gesture is required.
2.5.2 Pointer Cancellation (Level A 2.1 and 2.2)Conformance Level: SupportsRemarks and Explanations: Buttons on S5, S7 and S10 act on release. Moving the pointer off a button before release cancels the action.
2.5.3 Label in Name (Level A 2.1 and 2.2)Conformance Level: Partially SupportsRemarks and Explanations: S2: the button labelled "Search" has the accessible name "Find programs", so a speech command to click Search fails. Other visible labels match their names.
2.5.4 Motion Actuation (Level A 2.1 and 2.2)Conformance Level: Not ApplicableRemarks and Explanations: No function on the web screens or in the iPhone app is triggered by moving the device.
3.1.1 Language of Page (Level A)Conformance Level: SupportsRemarks and Explanations: All web screens declare lang="en". NVDA uses the English voice for the page content.
3.2.1 On Focus (Level A)Conformance Level: SupportsRemarks and Explanations: Moving focus with Tab causes no change of context on any sampled screen.
3.2.2 On Input (Level A)Conformance Level: Partially SupportsRemarks and Explanations: S2: changing the Season select reloads the list and moves focus to the top of the page with no prior notice. Other selects act only after pressing Apply.
3.3.1 Error Identification (Level A)Conformance Level: Does Not SupportRemarks and Explanations: All five forms in the sample (S4, S5, S6, S8, S10) show errors in red text only. NVDA announces nothing and focus stays on the submit button. Example, S10 payment: a card error appears above the form and is not tied to the field.
3.3.2 Labels or Instructions (Level A)Conformance Level: Partially SupportsRemarks and Explanations: S10: the expiry field's only label is the placeholder MM/YY, which disappears on typing. Other fields have visible labels.
4.1.1 Parsing (Level A)Conformance Level: SupportsRemarks and Explanations: W3C's note in WCAG 2.1 says this criterion should be considered always satisfied for content using HTML or XML. The web screens use HTML.
4.1.2 Name, Role, Value (Level A)Conformance Level: Partially SupportsRemarks and Explanations: iPhone app, S15 class list: the favorite star is read as "button" with no name by VoiceOver. S13: date cells do not expose the selected state. Standard controls on other screens are exposed correctly.

Table 2: Success Criteria, Level AA

Fictional product. The rows illustrate format only.

Notes: rows cover the web screens S1 to S14 and the iPhone screen S15.

Table 2: Success Criteria, Level AA. Fictional product; the rows illustrate format only.
CriteriaConformance LevelRemarks and Explanations
1.2.4 Captions (Live) (Level AA)Conformance Level: Not ApplicableRemarks and Explanations: The product has no live audio or video.
1.2.5 Audio Description (Prerecorded) (Level AA)Conformance Level: Does Not SupportRemarks and Explanations: S1 welcome video, the only video in the product: there is no audio description track. The sign-up steps shown at 0:52 are not spoken. The transcript meets 1.2.3 at Level A but not this criterion.
1.3.4 Orientation (Level AA 2.1 and 2.2)Conformance Level: SupportsRemarks and Explanations: S2, S7 and the S15 iPhone screen work in portrait and landscape. Nothing locks the orientation.
1.3.5 Identify Input Purpose (Level AA 2.1 and 2.2)Conformance Level: Partially SupportsRemarks and Explanations: S5: name, email and phone fields have autocomplete values. S10: the card and billing fields have none.
1.4.3 Contrast (Minimum) (Level AA)Conformance Level: SupportsRemarks and Explanations: All text in the sampled screens measured at 4.5:1 or above. The lowest is 4.6:1 on the footer links.
1.4.4 Resize Text (Level AA)Conformance Level: SupportsRemarks and Explanations: At 200% browser zoom, content and functions remain available on S1 to S14.
1.4.5 Images of Text (Level AA)Conformance Level: SupportsRemarks and Explanations: The only image of text is the product logo, which is exempt. Headings and buttons are real text.
1.4.10 Reflow (Level AA 2.1 and 2.2)Conformance Level: Partially SupportsRemarks and Explanations: At 320 px wide, S13 cuts off Saturday and Sunday in the calendar with no way to scroll to them. The S14 roster scrolls sideways as a data table, which the criterion allows. Other screens reflow to one column.
1.4.11 Non-text Contrast (Level AA 2.1 and 2.2)Conformance Level: Partially SupportsRemarks and Explanations: S5: input borders are #B8B8B8 on white, measured at 1.9:1. Focus rings measured at 4.8:1 and the other control boundaries at 3:1 or above.
1.4.12 Text Spacing (Level AA 2.1 and 2.2)Conformance Level: SupportsRemarks and Explanations: With W3C's text spacing values applied, no text is cut off or overlapped on S1 to S14.
1.4.13 Content on Hover or Focus (Level AA 2.1 and 2.2)Conformance Level: Partially SupportsRemarks and Explanations: S3: the age-rule tooltip appears on hover, disappears when the pointer moves onto it, and cannot be dismissed with Escape. No other hover content was found.
2.4.5 Multiple Ways (Level AA)Conformance Level: SupportsRemarks and Explanations: Programs can be reached by site search, by browsing categories and from the site map.
2.4.6 Headings and Labels (Level AA)Conformance Level: SupportsRemarks and Explanations: Headings and labels on the sampled screens describe their topic or purpose, for example "Choose a session" on S7.
2.4.7 Focus Visible (Level AA)Conformance Level: Partially SupportsRemarks and Explanations: S7: the session radio buttons show no focus indicator. Other controls show a 2 px ring.
3.1.2 Language of Parts (Level AA)Conformance Level: Partially SupportsRemarks and Explanations: S8: the Spanish waiver paragraph is not marked lang="es", so NVDA reads it with the English voice.
3.2.3 Consistent Navigation (Level AA)Conformance Level: SupportsRemarks and Explanations: The main navigation and footer links are in the same order on all web screens.
3.2.4 Consistent Identification (Level AA)Conformance Level: SupportsRemarks and Explanations: Search and Save have the same names and icons wherever they appear in the sample.
3.3.3 Error Suggestion (Level AA)Conformance Level: Partially SupportsRemarks and Explanations: S5: an invalid email shows "Invalid" with no suggestion. S10: an expiry error gives the expected format.
3.3.4 Error Prevention (Legal, Financial, Data) (Level AA)Conformance Level: SupportsRemarks and Explanations: S10: a review step shows the amount before payment and allows going back. S11: a registration can be cancelled within 24 hours.
4.1.3 Status Messages (Level AA 2.1 and 2.2)Conformance Level: Does Not SupportRemarks and Explanations: S2: the changed result count after filtering is not announced. S7: the "Seat added" message is not announced. S12: the save message is not announced. No sampled status message reaches NVDA without a focus change.

Table 3: Success Criteria, Level AAA

Not Evaluated. The report does not cover Level AAA, so this table states that for all AAA criteria and lists no rows.

Accessibility of ExampleRec Registration

ExampleRec Civic Software wants people of all abilities to be able to register for parks and recreation programs online. We work toward WCAG 2.1 Level AA.

Version 3.4 was tested on the screens listed in our Accessibility Conformance Report. These parts have known issues: choosing a site on the map, form error messages, the picnic shelter calendar and status messages. Our roadmap gives a workaround and a target date for each.

If something does not work for you, write to accessibility@examplerec.example. Tell us the page and what you were trying to do. We answer within three working days. Staff can also complete a registration for you.

Last reviewed: 5 November 2026.
